Aging, nerve paralysis, pre-existing low brow anatomy, or other conditions can cause our eyebrows to droop. This lowering can result in a “heavy” eyebrow that creates an angry or tired appearance and can contribute to frown lines around the eyes or furrows across the forehead. In extreme cases, the eyebrows may droop so low as to interfere with vision.

What Is a Brow Lift?

A brow lift is a surgical procedure performed in order to elevate and stabilize the brows. Depending on the type of brow lift surgery performed, it can also address loose or wrinkled forehead skin.

Types of Brow Lift Surgery

The oculoplastic surgeons at the Dean McGee Eye Institute are trained in a variety of surgical approaches to correcting drooping eyebrows. The surgery chosen will depend on a number of variables, including current eyebrow and hairline positions, overall facial structure, underlying conditions, and your desired outcome. Your surgeon will discuss the options available to you which include both functional and cosmetic surgeries. The types of surgeries offered include:

• Internal Brow Lift – An internal brow lift, or browpexy, may be performed simultaneously with upper eyelid blepharoplasty or ptosis repair and uses the same incision. In this procedure, the brow fat that has descended into the eyelid is reattached to its original position further up the forehead. This procedure is best for achieving a subtle lift of the outside corner of the brow without an additional incision and minimal recovery time compared to other brow lift surgeries.
• Direct Brow Lift – A direct brow lift is performed using an incision placed directly above the eyebrow hairs to minimize scar visibility. The eyebrow may be significantly and durably elevated utilizing this technique and may be performed alone or in combination with additional eyelid surgeryies under local anesthesia with or without intravenous sedation. Permanent sutures are often utilized to minimize scarring and are removed after seven to 10 days.
• Endoscopic Brow Lift/Forehead Lift – This procedure requires three to five small incisions made in the scalp behind the hairline in order to minimize visible scarring. The surgeon then uses an endoscope (a thin, flexible instrument with a light and camera) to release and elevate the entire forehead, which may be secured using sutures or other fixation devices. The incisions are typically closed with staples which are removed after seven to 10 days. This procedure lifts and shapes the brow while also softening forehead wrinkles with subtle elevation of the hairline.
• Pretrichial Brow Lift/Forehead Lift – For patients with significant brow ptosis and a high hairline, a pretrichial brow lift may be the best option. An incision is made along the hairline between the temples, the forehead is released, excess skin is removed, and the forehead is secured with sutures in order to elevate and shape the eyebrows. This procedure may restore a more youthful eyebrow position with additional improvements in eyelid position while smoothing forehead wrinkles. The incision typically camouflages nicely along the hairline with a subtle descent of the hairline profile.
• Coronal Brow Lift/Forehead Lift – For patients with significant brow ptosis and a low hairline, a coronal brow lift may be the best option. This procedure is similar to a pretrichial lift, but the incision is made 2-3 centimeters behind the hairline, the forehead and brows are released and raised, and then the incision is closed with staples which are removed after seven to 10 days. The goal is to raise the eyebrows, smooth the forehead and, lower the hairline subtly.

This beautiful lady had a prior blepharoplasty (eyelid skin removal) from another physician, but still felt the area was somewhat heavy. We offered and proceeded with an endoscopic forehead and brow lift to be able to reset her brow position but hide all her scars behind her hairline. Recovery from Brow Lift Surgery

Recovery will depend on many factors, including what type of surgery was performed and whether or not other procedures were performed in conjunction with your brow lift.

In general, you can expect bruising, swelling, and tenderness following surgery. Some patients undergoing forehead lifting in addition to brow lifting may experience temporary headaches, numbness, or paresthesia (a burning or prickling sensation) which may be treated with medication prescribed by your physician. Most patients can return to normal activities in two to three weeks.

Your surgeon will discuss with you in detail what you can expect in terms of recovery. For maintenance of cosmetic interventions, botulinum toxins can be utilized with a personalized treatment map to prevent excessive dynamic movement and earlier recurrence of wrinkles.
