Sắt cần cho hoạt động của nhiều loại protein đồng thời tham gia vào các phản ứng oxi hóa khử, kiểm soát quá trình sản xuất năng lượng, hô hấp của ty lạp thể và tổng hợp ADN. Tổng lượng sắt trong cơ thể bằng 0,008% trọng lượng cơ thể.
Trong cơ thể, sắt được phân bố thành 3 khu vực: chức năng, vận chuyển và dự trữ. Khu vực chức năng: Khoảng 2/3 lượng sắt trong cơ thể ở trong khu vực chức năng, chủ yếu trong hemoglobin. Một gam hemoglobin chứa 3,3 mg sắt, một mililit khối hồng cầu đậm đặc có một mg sắt. Lượng sắt trong myoglobin rất thấp nhưng có trong tất cả các tế bào cơ xương và tim. Một lượng rất nhỏ sắt chức năng (6-8 mg) ở trong cytochrome và enzyme, đặc biệt có trong enzyme ribonucleotit reductase. Vì thế, sắt có vai trò trong quá trình chuyển hoá của mọi tế bào. Trong huyết tương, sắt được vận chuyển dưới dạng Fe3+ gắn với transferrin. Vai trò của transferrin là hoà tan và gắn với Fe3+ ở dạng sinh lý (tránh để sắt ở dạng tự do) và vận chuyển cung cấp sắt cho tế bào thông qua các thụ thể gắn sắt. Khu vực dự trữ: Khoảng 30% lượng sắt của cơ thể ở dạng dự trữ, trong đó 60% ở gan và 40% ở hệ võng nội mô. Tại gan, trên 95% sắt dưới dạng ferritin trong tế bào gan, phần còn lại trong tế bào Kupffer dưới dạng hemosiderin. Còn ở lách và tủy xương thì sắt chủ yếu được dự trữ tại các tế bào liên võng nội mô. Sắt chiếm 20 - 25% trọng lượng phân tử ferritin. Ferritin là nguồn cung cấp sắt để tổng hợp hemoglobin trong hồng cầu. Khi hồng cầu tăng nhu cầu tổng hợp hemoglobin, lượng sắt trong nội bào cũng như lượng sắt trong phân tử ferritin giảm đi. Ferritin tự do trong huyết thanh phản ánh nồng độ sắt dự trữ. Nồng độ ferritin tăng cao trong các trường hợp cơ thể thừa sắt. Ngoài ra ferritin tăng cao còn gặp trong các trường hợp có khối u, viêm cấp và mạn tính. Hemosiderin là một phức hợp sắt - protein, được tạo ra từ ferritin, khoảng 10% ferritin có khuynh hướng hình thành hemosiderin, có thể nhìn thấy được hemosiderin dưới kính hiển vi quang học sau khi nhuộm với ferrocyanure de potassium (Perls). Hemosiderin là sản phẩm cô đặc dạng bán tinh thể của ferritin tập trung chủ yếu trong gan, lách, tủy xương. Trong trường hợp thừa sắt, lượng hemosiderin có thể được tích luỹ cao tới gấp 10 lần ferritin. Sắt trong hemosiderin khó được giải phóng hơn sắt trong ferritin. Phần lớn chuyển hoá sắt được thực hiện trong hệ thống khép kín giữa các khu vực với nhau. Ở người trưởng thành, 95% nhu cầu sắt để tạo hồng cầu được tái sử dụng từ quá trình phân huỷ hồng cầu già, chỉ có 5% lượng sắt được lấy thêm từ thức ăn. Cơ thể chỉ cần cung cấp 1 mg sắt/ngày là đủ cho nhu cầu tạo hồng cầu bình thường. Quá trình tiêu hoá và hấp thu sắt bắt đầu ở dạ dày nhưng chủ yếu tại hành tá tràng và đoạn đầu hỗng tràng. Sự kiểm soát quá trình hấp thu sắt và lượng sắt được vận chuyển vào máu tĩnh mạch cửa phụ thuộc vào nhu cầu sắt và kho dự trữ sắt của cơ thể. Trong trường hợp cơ thể quá tải sắt, lượng sắt được hấp thu vào tế bào biểu mô ruột sẽ giảm đi. Vận chuyển và sử dụng sắt: Trong suốt quá trình tế bào hồng cầu già sinh lý, cấu trúc màng tế bào bị thay đổi, dễ gắn kết với các IgG, là tín hiệu cho các đại thực bào ở gan và lách đến thực bào. Mỗi ngày có khoảng 1/120 số lượng hồng cầu bị thực bào, tạo ra khoảng 16,5 - 20 mg sắt, lượng sắt này được giải phóng vào máu, được transferrin vận chuyển đưa đến tủy xương để tạo hồng cầu mới. Khi phức hợp transferrin - Fe3+ đi đến tế bào, Fe3+ gắn vào transferrin receptor trên bề mặt tế bào đích và được vận chuyển vào trong tế bào. HFE là một protein tham gia điều hòa chuyển hóa sắt, HFE cạnh tranh với transferrin - Fe3+ tại vị trí TfR1. Phức hợp HFE - TfR1 ngăn cản không cho TfR1 gắn với transferrin - Fe3+, do vậy sắt sẽ không thể được vận chuyển vào trong tế bào. Tuy nhiên, HFE không gắn với TfR2, nên TfR2 có thể vận chuyển không giới hạn sắt vào trong tế bào gây tình trạng thừa sắt trong tế bào như ở tế bào gan, tim, tuyến nội tiết. Trong tế bào, sắt được chuyển đến ty lạp thể, tại đây sắt được gắn vào protoporphyrin để tổng hợp hem hoặc dự trữ trong ferritin. Ferroportin là một protein vận chuyển sắt xuyên màng tế bào, ferroportin có trong tế bào biểu mô đường tiêu hoá, tế bào gan, tế bào kupffer và đại thực bào. Trong cơ thể, hormone hepcidin điều hòa hoạt động của ferroportin. Điều hoà chuyển hoá sắt trong tế bào: Chất quan trọng nhất trong điều hòa chuyển hóa sắt là hepcidin, do gan sản xuất. Hepcidin điều tiết sự hấp thu sắt của các tế bào niêm mạc ruột và điều tiết quá trình giải phóng sắt của các đại thực bào. Hepcidin ức chế 23 ferroportin bằng cách gắn và giáng hóa ferroportin, dẫn đến giảm hấp thu sắt từ thức ăn vào tế bào biểu mô đường ruột. Hepcidin ức chế giải phóng sắt từ đại thực bào. Ở người bình thường, khi cơ thể thiếu sắt, gan giảm tổng hợp hepcidin. Khi đó ferroportin được giải phóng, vận chuyển sắt từ thức ăn vào biểu mô ruột, rồi vào hệ tĩnh mạch cửa. Hepcidin giảm, đại thực bào tăng giải phóng sắt dự trữ trong ferritin. Khi cơ thể thừa sắt, gan tăng tổng hợp hepcidin, dẫn đến giảm ferroportin. Ferroportin giảm, làm tế bào biểu mô đường ruột giảm hấp thu sắt. Hepcidin tăng, đại thực bào hạn chế giải phóng sắt dự trữ trong ferritin. XEM THÊM: Ferroportin-1, also known as solute carrier family 40 member 1 (SLC40A1) or iron-regulated transporter 1 (IREG1), is a protein that in humans is encoded by the SLC40A1 gene, and is part of the Ferroportin (Fpn) Family (TC# 2.A.100).[1] Ferroportin is a transmembrane protein that transports iron from the inside of a cell to the outside of the cell. Ferroportin is the only known iron exporter.[2] n/a n/a n/a n/a n n/a n/a n/a n/a n/a After dietary iron is absorbed into the cells of the small intestine, ferroportin allows that iron to be transported out of those cells and into the bloodstream. Fpn also mediates the efflux of iron recycled from macrophages resident in the spleen and liver.[3] Ferroportin is regulated by hepcidin, a hormone produced by the liver; hepcidin binds to Fpn and limits its iron-efflux activity, thereby reducing iron delivery to the blood plasma.[4] Therefore, the interaction between Fpn and hepcidin controls systemic iron homeostasis.
Members of the ferroportin family consist of 400-800 amino acid residues,[5] with a highly conserved histidine at residue position 32 (H32), and exhibit 8-12 putative transmembrane domains. Human Fpn consists of 571 amino acid residues.[5] When H32 is mutated in mice, iron transport activity is impaired.[6] Recent crystal structures generated from a bacterial homologue of ferroportin (from Bdellovibrio bacteriovorus) revealed that the Fpn structure resembles that of major facilitator superfamily (MFS) transporters.[7][8] The prospective substrate binding site is located at the interface between the N-terminal and C-terminal halves of the protein, and is alternately accessible from either side of the cell membrane,[8] consistent with MFS transporters.
Ferroportin-mediated iron efflux is calcium-activated; studies of human Fpn expressed in Xenopus laevis oocytes demonstrated that calcium is a required cofactor for Fpn, but that Fpn does not transport calcium.[8] Thus, Fpn does not function as an iron/calcium antiporter. The thermodynamic driving force for Fpn remains unknown.
In addition to iron, ferroportin has been shown to transport cobalt & zinc,[9] as well as nickel.[8] Ferroportin may also function as a manganese exporter.[10] Ferroportin is found on the basolateral membranes of intestinal epithelia of mammals, including:[11][12]
Ferroportin-1 plays an important role in neural tube closure and forebrain patterning.[13] Mouse embryos lacking the Slc40a1 gene are aborted before gastrulation occurs, suggesting that the Fpn1 protein encoded is necessary and essential for normal embryonic development.[11] Fpn1 is expressed in the syncytiotrophoblast cells in the placenta and visceral endoderm of mice at E7.5.[1][11] Further, several retrospective studies have noted an increased incidence of spina bifida occurring after low maternal intake of iron during embryonic and fetal development.[14][15] A study examining the consequences of several different mutations of the Slc40a1 mouse gene suggested that several serious neural tube and patterning defects were produced as a result, including spina bifida, exencephaly, and forebrain truncations, among others.[13] Given the findings of studies to date, there appears to be significant evidence that intact iron transport mechanisms are critical to normal neural tube closure. Furthermore, other experiments have suggested that Fpn1 product and activity is required along the entire anterior-posterior axis of the animal to ensure proper closure of the neural tube.[13] It is known that ferroportin (SLC40A1) gene is expressed at a low level in infertile women. Its mRNA levels were discovered to be down-regulated in these women, specifically in granulosa cells. What's more, low expression of ferroportin is also associated with infertility when some features like age and smoking habits are considered. It is also important to mention that, not only is ferroportin down-regulated in granulosa cells, but also in cervical cells of infertile women, and that the association between infertility and low ferroportin levels in these cells can be seen, again, when mRNA ferroportin levels was adjusted by age and smoking status.[16] Ferroportin is inhibited by hepcidin, which binds to ferroportin and internalizes it within the cell.[4] This results in the retention of iron within enterocytes, hepatocytes, and macrophages with a consequent reduction in iron levels within the blood serum. This is especially significant with enterocytes which, when shed at the end of their lifespan, results in significant iron loss. This is part of the mechanism that causes anaemia of chronic disease; hepcidin is released from the liver in response to inflammatory cytokines, namely interleukin-6, which results in an increased hepcidin concentration and a consequent decrease in plasma iron levels.[17] Ferroportin expression is also regulated by the IRP regulatory mechanism. If the iron concentration is too low, the IRP concentration increases, thus inhibiting the ferroportin translation. The ferroportin translation is also regulated by the micro RNA miR-485-3p.[18] Mutations in the ferroportin gene are known to cause an autosomal dominant form of iron overload known as type IV haemochromatosis or Ferroportin Disease. The effects of the mutations are generally not severe but a spectrum of clinical outcomes are seen with different mutations. Ferroportin is also associated with African iron overload. Ferroportin and hepcidin are critical proteins for the regulation of systemic iron homeostasis.
